Office policies and exclusions for Mint Dental Savers plan
– Membership is valid for 1 year from time of payment, and will continue on next payment 
– Membership fee is non-refundable
– Benefits do not accumulate if not used within membership term
– All treatment provided must be paid for at the time of treatment 
– The Savers plan cannot be used with any medical aid or other discount plans 
– Our office reserves the right to terminate any membership or refuse treatment without notice if member does not settle any accounts due   
– Dentures, crowns and bridges, tooth extractions (non-surgical), Dry socket treatment and tooth jewelry excluded from discount
